Prior to entry, attendance or transfer to preschools, schools (K-12), licensed child care facilities, and family day care homes, each child shall have on file a Florida Certification of Immunization, DH680 documenting the following:

A.Public/Non-Public Schools K-12
       Five doses of diphtheria, tetanus, and pertussis vaccine
       Four doses of polio vaccine
       Two doses of measles vaccine
       One dose of mumps vaccine
       One dose of rubella vaccine
       Three doses of hepatitis B vaccine
       Two dose of varicella vaccine (effective school year 2001/2002) Grades K - 2

B.Public/Non-Public Preschool (age-appropriate number of doses as are medically indicated)
       Diphtheria, tetanus, and pertussis vaccine
       Polio vaccine
       Measles vaccine
       Mumps vaccine
       Rubella vaccine
       Hepatitis B vaccine (effective 2001/2002 school year)
       Varicella vaccine (effective school year 2001/2002)
       Haemophilus influenzae type b (Hib) vaccine

C.Public/Non-Public Schools Seventh Grade
       1.Effective with the 1997/1998 school year, children entering, attending, or transferring to the seventh
          grade in Florida schools are required to complete the following:

        Second dose of measles vaccine
        Three doses of hepatitis B vaccine
        Tetanus-diphtheria booster (Td)



  
Immunization Requirements
Your child/ren will not be admitted to school in August if the proper forms (original HRS Immunization form #680 and HRS Physical form #3040, no copies) are not in his/her file.

IT IS THE PARENTS’ RESPONSIBILITY TO SEE THAT THE
STUDENTS IMMUNIZATIONS ARE UP TO DATE.
